- Cycle status → radar → radio once—Radar & Status and Terminals & Radio.
- Stand up and re-sweep the room you were ordered to inspect—Facility Exploration.

Confirm the build first

If progression died after you re-enabled or re-triggered an electrical puzzle, update. Pre-patch, that path could trap the mission state. Post-patch, re-test the puzzle calmly; if it still fails, document whether it was a first enable or a re-enable when you file a report.
Dimitri leg injury → death

Dimitri death flag mismatch
Sometimes the story treated Dimitri as dead while systems did not. That flag bug is called out in 1.23.1. Symptoms can look like later events ignoring an obvious death, weird UI, or branches that never open. After updating, replay from before the injury beat if a save is dirty.
Sticky VHS / media focus
